IMPORTANT
- Only one item or service may be requested per pre-approval submission.
- You must include a link to the appropriate page of the website for the item or service you are requesting.
- If an expense appears as "approved" on the Expense List, you are not required to seek pre-approval.
- You do not need to include taxes and shipping in your pre-approval request form. Only the cost of the item is required.
Please complete all required fields on this form. Be prepared to include the cost of the item requested. An approximate cost will be accepted, but an expense submitted in ClassWallet for an amount greater than the cost approved on this request form may result in a denial of your transaction. Upon submission, the pre-approval request will be processed by a member of the Utah Fits All Customer Service Team within 7 to 10 business days. You will not be able to submit this form until you agree with all the attestations. Please review the terms of the Utah Fits All Scholarship Program prior to completing this form.
A pre-approval of a qualifying expense is not a pre-approval of the transaction. Sufficient funds must be available in the account for the expense to be approved. The approval of this request form is valid for the 2024-25 school year only. Upload the pre-approval email and receipt of payment when requesting reimbursement. This will help expedite your request.
Utah Code Annotated § 53F-6-404(7) establishes a process for a scholarship student or a scholarship student’s parent to appeal any administrative decision of the Program Manager, including determinations regarding enrollment eligibility or suspension/disqualification from the program, or scholarship expense denials. Such appeals will be referred to ACE Scholarships and the appeals committee for review and resolution within 30 days after the day of the appeal. You must complete the form below within 10 days after the administrative decision is made if you want to start the appeal process.
OVERVIEW
ACE Scholarships, the Utah Fits All (UFA) Scholarship program manager, is required by law to provide a method for families to appeal any adverse administrative decision made by the program manager. Families may not appeal any program guidelines that are outlined in the UFA law. You must complete the form within 10 business days of receiving the administrative decision. Appeals will be reviewed and resolved within 30 calendar days from the date the appeal is filed.

ACE Scholarships has a committee (including parents of UFA scholarship students) to handle appeals. To thoroughly review your appeal, personal data may be shared with the UFA Appeals Committee and others who need to see it as required by law.
YOUR RIGHTS
- You can request access to and correct your data within 3 calendar days from the date of your appeal.
- You cannot request your data be transferred to another party.
- ACE Scholarships might deny access to some data if required by law.
- You can withdraw consent anytime, but data already shared can still be used as described.

In the absense of a birth certificate, parents or legal guardians must provide reliable proof of the student's identity and age along with this affadavit explaining the inability to produce a copy of the birth certificate.Affadvit and subsequent documentation upload must be completed no later than April 15. If the parent fails to complete this process, the application will be consdered incomplete and the student will be considered ineligible to participate in the program.

Utah Fits All Scholarships are awarded with preference toward students in the greatest financial need. To ensure your application is given the correct preference, it is important that you provide all of your 2023 income information. Failure to do so may lead to the suspension of your Utah Fits All Scholarship account. Applications without a verified household income will be assigned to the lowest priority group. Skipping this step could reduce your chances of being awarded a Utah Fits All Scholarship. All information in this form must be submitted to later than April 15, 2024 for consideration.

There was an issue identifying your residency. If you are a resident of Utah, you must provide reliable proof of residency. Active duty military personnel will be required to provide documentation to prove active military status. This documentation must be uploaded no later than April 15 in order for your application to be considered complete. If you fail to complete this process, the application will be considered incomplete and your student(s) will be considered ineligible to participate in the program.
