This answer depends on the type of school that you are.
Private schools with an enrollment of 150 or more students must provide:
- A federal employer identification number
- Tuition and fee schedule
- School name, address and website
- Parent point of contact information
- An agreed upon procedures engagement conducted by independent licensed certified public accountant OR an audit report that: (A) a licensed independent certified public accountant conducts in accordance with generally accepted auditing standards; (B) presents the financial statements in accordance with generally accepted accounting principles; and (C) audits financial statements from within the 12 months immediately preceding the audit
Each private school must sign an attestation agreeing to the rules and requirements of the Utah Fits All Program.
A private school that enrolls 150 or more students is not eligible to receive scholarship funds if the private school requires a scholarship student to sign a contract waiving the scholarship student's right to transfer to another qualifying provider during the school year or the audit report contains a going concern explanatory paragraph; or the report of the agreed upon procedures shows that the private school does not have adequate working capital to maintain operations for the first full year.

- School name, address and website
- Parent point of contact information
- A tuition and fee schedule
Each private school must sign an attestation agreeing to the rules and requirements of the Utah Fits All Program.
A private school that enrolls less than 150 students is not eligible to receive scholarship funds if the private school requires a scholarship student to sign a contract waiving the student's rights to transfer to another qualifying provider during the school year.
